Shouldice repair technique

What are the procedures in hernia surgery? Could mesh sutures reduce incisional hernias? What surgery is used to correct inguinal hernias? A recurrence rate around has been demonstrated by our group and many others over the years. The principle of the technique is based on more than the technical aspect alone and includes the preoperative preparation, anesthesia, comprehensive inguinal dissection and postoperative principles.

Then a second running suture is used to plicate the oblique muscles, creating a four-layer hernia repair.

This approach improves recovery, and ensures that our patients are comfortable during surgery. The transversalis fascia is incised from the internal ring laterally to the pubic. A teaching film by Sue and Peter Jones, Specialist Hernia Surgeons in Maidstone, Kent UK. The repair is based on the Bassini repair with some modification.

The Bassini repair , named after Dr. This unique laminated closure of a hernia allows the repair to be performed without tension and under local anesthesia. David Grischkan, The Hernia Center of Ohio’s. The use of mesh is associated with a lower rate of recurrence.

The quality of included studies, assessed with jaded scale, were low.

This repair uses a patients natural tissues to repair the weakness in the abdominal wall. Reinhorn was first asked by a patient to perform this technique. Bassini-type repair approximating transversus abdominis aponeurosis and transversalis fascia to iliopubic tract. Recently a new tissue repair technique has been proposed by Desarda and studied in trials against Lichtenstein technique. The first method of this type of repair is known as tension hernia repair.

The Method of Tension-Free Hernia Repair. A small incision is made into the abdominal wall under local anesthesia. Shouldice technique during his surgical training in Canada.

Thanks for the info, TT. Satava is a hernia specialist? Annual meeting of the Ontario Medical Association, District number Sept 1. It was named after E. It still specializes in the procedure. That means it places tension on muscle tissue around the hernia.

Complications after a mesh surgery can drive the cost of a simple hernia repair into the hundreds of thousands of dollars. These flaps are then overlapped (double-breasted) with two layers of sutures. Nearly everyone does mesh repair. I watched several surgery videos online.

In the evaluation of postoperative complications, this technique , which has the closest to Lichtenstein herniorrhaphy, could be the preferred method in selected cases of primary inguinal hernia repair.

Open hernia repair is tried and true. It has been performed for over a century and there are close to a hundred different techniques that can be employed. The gold standard open repair is with mesh, with the Lichtenstein technique being the most common with the best outcomes.

In the open category, repair of the hernia is achieved by pure tissue approxi-mation or by tension-free mesh repair. Instead of approximating the transversalis fascia and the aponeurotic margin of the transverse abdominal muscle to the iliopubic tract and to Poupart’s ligament to repair either a direct or indirect hernia, the McVay repair attaches these musculotendinous structures to Cooper’s ligament and the lacunar ligament medially and the inguinal ligament laterally.
